Modifier 58: Staged or related procedure by the same physician during the postoperative period
When it is appropriate
Append modifier 58 to a procedure performed during another procedure's postoperative global period when it was planned or anticipated at the time of the original surgery (staged), is more extensive than the original, or is therapy following a diagnostic surgical procedure, by the same physician.
When it is not appropriate
Do not use 58 for an unplanned return due to a complication (use 78 instead) or for a procedure that is genuinely unrelated to the original surgery (use 79 instead).

58 is one of the global surgery modifiers CMS lists as able to bypass an NCCI PTP edit specifically for planned, staged, or more extensive related procedures.
