Modifier 78: Unplanned return to the operating or procedure room for a related procedure during the postoperative period

When it is appropriate

Append modifier 78 to a procedure code when the patient has an unplanned return to the operating or procedure room for a complication of the original surgery during its postoperative global period, performed by the same physician.

When it is not appropriate

Do not use 78 for a planned, staged return already anticipated at the time of the original surgery (use 58 instead), or for a return that is unrelated to the original procedure (use 79 instead).

78 is one of the global surgery modifiers CMS lists as able to bypass an NCCI PTP edit specifically for this related, unplanned return to the operating room scenario.
