Modifier XP: Separate practitioner
When it is appropriate
Use XP when the column two service was performed by a different practitioner than the one who performed the column one service, even within the same group and specialty, on the same date.
When it is not appropriate
Do not use XP when the same practitioner performed both services, or to describe a supervising physician's involvement in a service actually performed by someone else without a genuinely separate service being rendered.
CMS preference
Use XP over 59 whenever practitioner identity is the reason the services are distinct.
